New Jersey healthcare consumers with Horizon Blue Cross Blue Shield, particularly those on Medicare, can once again turn to Meadowbrook, Pennsylvania-based Holy Redeemer for home care and hospice care under a newly reestablished relationship between the health system and the insurer, Holy Redeemer announced. The effective date is May 1.

Holy Redeemer Home Care and Hospice serves patients in Atlantic, Ocean, Mercer Middlesex and Union counties, among others, and historically has had a concentration of patients in Elizabeth, Camden, Trenton and other urban areas of the state. Adopting an integrated “Advanced Care Service Model,” Holy Redeemer is able to provide the right palliative and chronic illness care at the right time, with a focus on personal dignity and choice.
